Quality

  • Are you providing vaccinations and medications of the same quality standard you would expect for your own family? Are these documented appropriately?

  • Do patients receive an updated shot record & a current Vaccine Information Statement (VIS) with each vaccine? Is it documented in the medical record

  • Are your policies and procedures regularly reviewed and updated to align with current ACIP/CDC/VFC recommendations?

  • Are current immunization schedules posted in clearly visible areas throughout the facility?

  • Do you maintain a current emergency management plan that includes emergency contacts, thermometers information, storage units details, a maintenance log, emergency relocation location, and staff training?

  • Do staff receive regular training on urgent medication transport preparation protocols?

  • Do new staff receive comprehensive immunization training and injection technique observation? And ongoing updates and education at least annually?

Storage Units

  • Do you use only stand-alone refrigerator or freezer units, excluding dormitory-style or combination refrigerator/freezer units?

  • Is the unit free of glass shelves and freezing cooling plates on the back wall?

  • Are the unit floor, drawers, and doors kept clear of vaccines at all times?

  • Are perforated baskets used exclusively to ensure adequate air circulation?

  • Are shelves and baskets clearly designated and labeled by category (e.g., Private, VFC, State)?

  • Are food items and biological specimens kept separate from the vaccine storage unit at all times?

  • Do you maintain a reliable vaccine transport cooler equipped with a data logger for use during emergencies or supply relocations?

  • Is routine maintenance performed on the storage unit to ensure continued proper functioning?

Warning Signs & Labels

  • Are all supplies and storage units clearly labeled with CDC-recommended warning signs to minimize the risk of medication errors?

  • Are high-alert medication labels applied where required?

  • Are storage designation labels in place (e.g., Medications Only)?

  • Are product labels clearly displayed, including vaccine names and medication identifiers?

  • Are storage boxes labeled by category (e.g., Private, VFC)?

  • Are "Do Not Unplug" signs posted on: storage units, electrical outlets and circuit breaker?

  • Is a "Do Not Adjust Thermostat" sign posted on or near the thermostat?

  • Do you post "Protect From Light" signs to try to keep all medicines and vaccines covered in original packaging?

  • Are emergency contact labels and response protocols posted in a visible location?

  • Are open dates and discard dates labeled on all applicable products?

Thermometers

  • Are all vaccine storage units monitored with calibrated thermometers, and are certificates of calibration maintained on file?

  • Is one 24-hour digital data logger thermometer in use in each vaccine storage unit? If so, is the data downloaded and reviewed at least every two weeks, and is the device configured with the correct time, date, and sampling intervals?

  • Is the thermometer probe bottle positioned vertically and hanging (preferably)?

  • Are at least one backup data logger and one digital thermometer kept on hand?

  • Are temperatures monitored in the medication room and all other refrigerators?

  • Are thermometer battery replacements performed regularly and documented?

  • Is the replacement of thermometers documented?

Temperatures

  • Is staff monitoring and recording storage unit temperatures twice daily on a temperature log?

  • Are multiple staff members trained on handling newly received vaccine shipments, and are shipments promptly stored upon arrival?

  • Do staff know the proper procedures for responding to a temperature excursion?

  • Do staff know how to report temperature excursions, who to contact, and how to determine vaccine viability?

  • Are temperature logs retained for a minimum of three years?

  • Are several gallons of water kept in the refrigerator, and are frozen packs or water bottles kept in the freezer to support temperature stability and for emergency relocation?

  • Is the water clearly labeled "Do Not Drink"?

Patient Safety

  • Is staff labeling every syringe with its content label prior to administration?

  • Does a nurse or licensed provider verify syringes against the order before administration?

  • Are patients screened for possible contraindications prior to administration?

  • Are all supplies and shelves clearly labeled?

  • Are products rotated to ensure items expiring soonest are used first?

  • Are look-alike and sound-alike medications (e.g., Tdap, DTaP, Flu, Insulin) physically separated and clearly labeled?

  • Are multi-dose vials labeled with both the open date and discard date?

  • Are staff vaccinated against influenza and other vaccine-preventable diseases?

  • Do vaccinated staff members wear an influenza vaccination sticker, if this is adopted as a compliance measure?

  • Is a current emergency management protocol for adverse reactions maintained, along with an accessible emergency kit?
